Health Office Forms
All students at Rancho High School are required to submit form 768 (Health Information Card) every year. Due to distance learning, this will be updated by electronically provided in link. We update your child's medical information on a yearly basis.
If we return to in person education and your child has a chronic medical condition or is in need of taking medications at school, please complete forms 503, 643, and see the health office - for specific medical order forms. Medical order forms vary depending on your child's condition/needs.
