Proof of Address Documents
DOCUMENTS MUST BE CURRENT WITHIN THE LAST 30 DAYS PRIOR TO ENROLLMENT
The following documents may be used for Proof of Address
- Current mortgage statement, lease agreement or rent receipt
- Recent utility bill such as gas, water or electric - cable and telephone bills are not acceptable.
Residential affidavit
If the parent or guardian is living with a friend or relative, the parent or guardian must bring a notarized statement signed by the parent or guardian and the owner/ renter of the property indicating that it is their place of residence.
The owner/renter of the property must provide a government-issued photo identification (e.g., driver’s license, state-issued identification card, passport) that bears their name. Failure to provide identification may result in a withdrawal of the child from the school to which the residential affidavit was presented.
School enrollment using a residential affidavit is not a guarantee of athletic eligibility.
